What to send
- Your original, undamaged passport (a 10-year passport, expired no more than 5 years ago)
- A copy of your driver's license or state ID, if possible
- Two passport photos, 2 x 2 inches, on a white background
- A personal check or money order for $212.05, payable to the U.S. Department of State
- A travel itinerary if you are traveling within 14 days
- Form DS-82, completed online, printed, and signed
- Two Letters of Authorization of the same kind (check the first two options, leave the courier company blank)
- Do not mail anything until you have spoken with us first

Service Order Form
Print and complete the Service Order Form and include it with your documents.
Two Letters of Authorization
Fill out two letters of the same kind. Check the first two boxes only, and leave the courier company blank until you call us.
Personal check or money order
Make it payable to the U.S. Department of State. The adult renewal government fee is $212.05.
Travel itinerary
If you have a ticket or itinerary showing travel within 14 days, include a copy. If you do not, please call us.
Passport photos
Two photos, 2 x 2 inches, on a white background, taken within the last month. We take compliant photos in our office.
Changed your name?
If you changed your name through marriage or divorce, include a certified marriage certificate or court decree. Originals or certified copies only.
Proof of identity
Include a clear copy, front and back, of one valid ID:
- A valid U.S. driver's license or state ID
- A U.S. passport (it may be expired)
- A Certificate of Naturalization or Citizenship
- A government employee or military ID
- A valid foreign passport
Can you renew by mail?
You can renew by mail if your most recent passport:
- Is undamaged and can be submitted with your application
- Was issued within the last 15 years
- Was issued when you were 16 or older
- Was issued in your current name, or you can document a name change
