The damaged passport. Bring it with you, even if it is badly damaged.
Statement of damaged passport. We have the form and complete it with you.
Proof of citizenship. Original birth certificate or naturalization certificate.
Valid photo ID. State driver's license, state ID, or another government ID.
Government fee. A check with a printed address, a money order, cash, or a debit card.
Passport photos. We take them in our office, so there is nothing to bring.
More than normal wear? Water damage, a detached cover, or missing or unreadable pages all require a replacement, not a renewal.
